A picture proves nothing by itself: the same spot means one thing in somebody with no complaints and another in somebody who is ill. The word to carry away is el medio, in el medio de contraste yodado. Spanish asks it to do five jobs that English hands to five different words. It is the middle — en medio de la sala. It is the means — un medio de transporte. It is the medium, which is the sense here and in el medio de cultivo as well. It is the environment, in el medio ambiente. And as an adjective it is half: media hora. Meeting it cold, a reader has to settle which of the five before the sentence can go on.
