The twenty-third entry is o formulário de consentimento, and the first noun sets a trap that only springs inside a hospital. Formulário is a form, the sheet with boxes on it, and the English word for that is « form »: a consent form, an admission form, a claim form. « Formulary » looks like the obvious cousin and names a different object altogether. In English medicine a formulary is the list of medicines an institution has agreed to use, with the doses and the restrictions printed beside them; the national one is the reference book every prescriber in Britain owns, and a hospital keeps its own on top of it. So « the drug is not on the formulary » says that nobody may prescribe it there without special approval, and says nothing whatever about paperwork. The two words even pull in opposite directions: a form is empty until somebody fills it in, while a formulary arrives full and closed. Write « form » for formulário every time, and keep « formulary » for the book of medicines.
